Upmédica: Portable Digital Health Ecosystem

Founded in 2015, Big Health Data Consulting is a technology consulting firm in digital transformation, organizational culture, and regulatory compliance. Focusing on e-health, they design and develop digital health ecosystems, healthcare services, and training.

Focusing on the responsible empowerment of the patient, as well as the creation of services that improve the doctor-patient and doctor-doctor relationship, BHD has made possible the digital transformation of the Holy Trinity General Hospital Foundation (FHGST) of Salamanca, a reference hospital in Castilla León with more than 100 years of experience.
With Upmédica, their health technology platform, they have ensured that medical assistance has not stopped because of the coronavirus, even from home. The telemedicine tool has video consultation with doctors from a long list of specialties, access to medical records, monitoring by wearables, scheduling appointments, download results, access to prescriptions... All this with the guarantee of Azure, the cloud service of Microsoft, where Plain Concepts has been in charge of creating the structure to store all the data.

Challenges

Upmédica offers remote patient monitoring through medical devices and wearables (pulse oximeter, blood pressure meter, thermometer, heart rate, activity, sleep, and weight), facilitating the patient's control in real-time thanks to the data generated in these devices.
The big challenge was to host all this data in the cloud so that all parties can access and be constantly connected and communicate with other devices that further expand the spectrum of monitored variables. The patient had to access its functionalities from any connected device and did it simply, without complications, and from a modern environment. In terms of usability, both users and the medical team feel comfortable in a multi-device space. Also, security should be ensured and have a sound legal and technological basis in line with GDPR, LOPDGDD, and LSSI standards.

Results

An environment has been achieved in which medical teams are connected by digital interconnection, access to their patient's data, and the possibility of interacting with them through various care models, focusing on home hospitalization.

Besides, the patient can make consultations, both online and in-person, exchange medical documents with his or her usual professional, even if he or she is in another part of the world, download the prescriptions, or view the history of his or her consultations... And all this, in the same platform in which all the information is offered in the same simple and intuitive interface.

To host all this data, it was decided to use Azure, which has allowed the interconnection of all the parties involved, the sharing of information, and real-time access to all the data stored in the cloud. With a network of CDN servers and the Microsoft technology that we have implemented, a refined and optimized navigation and video conferencing experience is provided anywhere on the planet. Also, the platform guarantees data security and has a solid legal and technological basis, responding to the regulations mentioned above.

Highlights

The development of telemedicine platforms such as Upmédica implies creating an ecosystem scalable to any need, which guarantees the improvement of the communication between doctor and patient, regardless of the place or the circumstances in which it is located. The integration of a system of acquisition, monitoring, storage, and biosignatories registered directly on the user establishes a bridge for processing large amounts of data and, consequently, improving the analysis systems, prevention, and personalization of medical services.

A clear example is the data recorded by the Salamanca FHGST, which recovered 50% of outpatient consultations during confinement, in addition to a 1,000% increase in the number of patients registered. This is a before and after in telemedicine because this platform is a great ally to bring added value to the medical service, especially during a health crisis like the current one, but it is not the only thing. It lays the foundations for the medicine of the future, as this comprehensive health solution is capable of ensuring the safe and efficient management of medical information and providing an uninterrupted, out-of-location and cost-saving care service, both in money and in time.

The projection of this project goes further, and whose next step will be the continuous monitoring that allows medical teams to analyze the monitored parameters without the need for the patient to make point measurements. The proposal is that the devices be carried over, automating the measurements, uploading that data to the platform, and monitoring them based on intelligent triage, based on the parameters defined by the medical equipment, prioritization of remote control, and care for hundreds or thousands of users. All this, launched using Microsoft Azure cognitive services.
